The figures are staggering! Approximately half of the population DOES NOT have or visit a dentist on a regular basis. There are many reasons people don’t seek regular dental care, but the most common reasons we hear – COST and NO INSURANCE. This is a huge concern for Dr. Zehnder and her team at Lifetime Smiles. We care deeply about our patients! The Lifetime Smiles Dental Care Plan offers uninsured clients and clients who once thought dental care was too expensive a wonderful option to put value and health back in their teeth!
Enrollment Fee & Coverage
What Is Included In The Enrollment Fee:
- Clients initial comprehensive exam, initial set of x-rays & routine cleaning* (valued at $427)
- Clients exam & routine cleaning* appointment at your next 6-month appointment (valued at $184)
- Additional diagnostic x-rays that might be needed for diagnostic purposes throughout the year at the hygiene visit, emergency visits and x-rays with our restorative team are subject to the 15% discount
- Enrollment into the Lifetime Smiles Dental Clare Plan the clients will receive 15% discount for ALL dental treatment.
- It cannot be combined with any other dental benefit coverage
*If you require a deep cleaning (Scaling & Root Planing or nonsurgical periodontal therapy) for gum disease, then note that procedure is not included in the enrollment fee. This treatment is subject to the 15% discount.
Terms And Conditions
- Enrollment fees are non-refundable once any treatment is performed. This includes initial exam, initial set of x-rays and cleaning. Payment in full is due on the date of service.
- Payment plans for future dental treatment are available for clients who qualify through Care Credit.
- Clients have the option of automatically renewing Lifetime Smiles Dental Care Plan and enrollment fee annually with credit card on file.
- Referrals to specialists for treatment may be necessary from time to time and their charges are not included in this membership plan
